Notice. All notices, demands or requests that are required or permitted to be given pursuant to this Agreement must submitted in writing. Notices must be delivered personally, by commercial carrier, by fax with a machine generated confirmation sheet or by registered or certified mail, postage prepaid, addressed to a party as stated below:
To Employer: Health Care Facility
Doctor Doctor, MD
With a copy to: Law Firm
ATTN: Lawyer Lawyer
To Employee: Doctor Physician, MD
Notice given personally or by commercial carrier is effective upon delivery. Notice given by electronic mail or by fax, with a machine generated confirmation sheet, is effective upon sending. Notice given by United States mail is effective the third (3rd) United States Post Office delivery day after the date of mailing.
